I have huge menstrual cramps that make me nauseous and make it difficult to move around. I don't have a family doctor or gynecologist. Who do I go to?

I had an IUD inserted 2 1/2 years ago. I've always had good pain, but never as bad as I do now. My cycle has never been regular (not menstruating for 3 months, menstruating every other week for 4 months, a full month menstruating....). 2 years after I got my IUD, I went to a private gynecologist ($300) to have her tell me that it's okay. But I can't live with the pain anymore. I don't take any medication

Usually, anti-inflammatory drugs such as ibuprofen (Advil, Motrin and other brands) or naproxen (Aleve) are the best options if you have no contraindications to taking them (which your pharmacist will confirm). Non-pharmacological measures such as heat application can also help.
If this is not successful, I believe that a reassessment of your condition will be necessary.
