Do I have a risk of getting pregnant even if my Nuvaring has never stayed off me for more than 3 hours?

My breasts have been hurting since this morning. I haven't done anything special. I take Nuvaring continuously. I am following the instructions well. I have never taken it out for more than 3 hours in a 24-hour period. The only time I take it out is for sex because it creates discomfort, but it is put back in immediately afterwards. I am on my 12th day of a 25 day cycle. What could it be? I am concerned.
