My daughter has 2 ear infections and has been taking the banana medicine since Wednesday noon. The fever stopped and she was better on Thursday (more cheerful, but eating very little). However, she had a fever again last night at 40.2.

We usually give 72 hours to evaluate the effectiveness of the antibiotic. I suggest continuing the treatment and combining it with Tylenol or Tempra if necessary. If the fever persists 72 hours or 72 hours after starting the treatment, you will have to consult again to reassess the diagnosis
If your child is 6 months old or less, I advise you to consult without delay.
