What could be the possible reasons for these fever outbreaks?

My 2 year old boy has been having fever spikes up to 40C over several days with no apparent cause... the spike often happens at night... I'm going to consult tomorrow but what could be the reason?

How is his sleep generally?
How is his general condition during the day?
Does he have any other symptoms? Earache? Nasal secretions? Coughing?

No other symptoms..
he sleeps less well because he is thirsty because of the temperature and his condition during the day is rather energetic but calmer and more grumpy than usual

If no other symptoms are apparent at this time, you did the right thing by making an appointment with a doctor. We can tell from your description that something is affecting his overall condition.
