Anusol Plus ointment can help relieve discomfort. It is available without prescription and is safe during pregnancy. It can be applied externally (and internally) twice a day and after each bowel movement if needed.
Is this due to constipation or diarrhea?

This is unfortunately a common problem for pregnant women.
In addition to the ointment, taking PEG-3350 (LAX-A-DAY, Restoralax and other brands) can help soften the stool. This is a safe product to take every trimester. I also encourage you to eat lots of fiber (fruits and vegetables, whole grains, etc) and drink lots of fluids during the day to soften stools.
