At what temperature is a 4 month old baby considered to have a fever?

Hello, For the last two-three days, my 4-month-old baby's temperature has been varying between 38 and 38.3. I have consulted several websites to know at what temperature a baby has a fever. Some say 38 and others 38.5. Do I give tylenol from 38? Please note that my baby's temperature is taken rectally. Thank you
