My 14 year old son has a headache and blurred vision. Should I seek help?

He already had a concussion 1 1/2 years ago...Since then he had no more symptoms but since 3 days he started to have headache and blurred vision again...he even had a strong pain in his chest 4 days ago but since then he just had headaches and blurred vision.he is a fit boy who does sports everyday,no drugs or cigarettes,not to my knowledge.
