Hello, I have a difficulty to swallow with swollen tonsils, the right one much more than the left one, big and painful ganglions, a pain in the right ear and fever. It's been more than 4 days now and in spite of the anti pain and anti inflammatory drugs the pain is increasing.

Good morning, Madam,
In general, the symptoms you describe may be indicative of an ongoing infection, so you may want to start by trying the steps listed here.
Some measures if you are not pregnant:
Take Tylenol and Advil for pain and inflammation
Eat cold foods like ice cream
Drink plenty of water and take honey to soothe the throat
Anesthetic lozenges to reduce pain
Gargle with water and salt (1/4 to 1/2 teaspoon of salt in 250 ml of water)
Rest as much as possible
On the other hand, if your symptoms get worse or you are not able to eat (even for water) in the next 2 days despite these measures, I suggest you go see a doctor.
I hope I have answered your question,
