You did the right thing by asking your question here. Indeed, the spironolactone you are taking for your acne causes, in about 10% of patients, gynecomastia. This is the scientific term for breast growth in patients. Therefore, it may very well be that the presence of sore and tender nipples is related to the use of this medication.
I recommend that you consult your pharmacist to indicate the presence of these symptoms and thus see about possibly modifying your acne treatment.
