My 16 month old daughter has had a plaque/blemish in her groin for over a week. It is itchy. What should I do?

It may be due to the presence of irritation from the moisture and rubbing of the diaper. As you said, the first thing to do is to leave it without a diaper as long/often as possible. Then, you can apply a 0.5% hydrocortisone cream to the skin twice a day. This will decrease the redness and itching.

If we see that the redness increases upon application of this cream, we will go to a doctor quickly.
