I would like to know if it is correct that I restarted my anticoagulant xarelto because I did not have a note or paper that told me to continue to stop? my pharmacist told me to restart too.

Hello,
In my experience, it is usually the surgeon who decides when to restart anticoagulation. Most of the time, if there were no complications during the operation, we restart the antigoagulants such as Xarelto the same night of the operation or the next day.
Kidney stones are usually removed without making an incision through the urinary tract. The risk of bleeding related to the operation therefore seems to me to be rather low and a quick resumption of Xarelto seems indicated.
Like your pharmacist, I would recommend restarting the medication, but I regret that you were not given clear instructions at the hospital.
Some blood in the urine over the next few days (or until the double D is removed, if necessary) may be normal. You should seek medical attention if you experience heavy or increasing bleeding.
Hope this helps,
Charles
