Am I using the wrong product? Should I consult a doctor?

My 6 weeks old baby has some small white pimples and is red in the crack of the buttocks. I apply Penaten Dupuis for a few days but it doesn't get better.

It is difficult to know for sure without seeing the lesions (impossible on this platform) but it is common that it is a fungal infection. It is common in the crack of the buttocks because it is a more humid area. If also more frequent if your child wears synthetic diapers (not cotton).
This being said, it can be treated well with an antifungal such as Clotrimazole (Canesten). Applying the cream twice a day and keeping the area as dry as possible (easier said than done... 😬) it should be much better in a few days. The important thing with this kind of treatment is to apply for 7-10 days because the infection tends to come back repeatedly if you don't get rid of it.
That said, Penaten is a good barrier cream that helps prevent erythema caused by friction (by the diaper). You can continue to apply it 15-20 mins after applying the antifungal to help protect the sensitive skin.
Lastly, and I know it's not easy, but you can try leaving the child without a diaper for a few times a day to try to keep the area drier :)
