My son has a very sore throat and has had a temperature for over 48 hours and when he coughs he cries. Can a pharmacist prescribe something or should I go to the emergency room?

Besides the fever and cough, does he have any other symptoms such as a stuffy or runny nose?
Have you tried anything for that?

Yes runny nose I give natural syrup I also do nasal douches

You did the right thing with the nasal douches, I encourage you to do them (followed by the baby fogger depending on the age of your child) as often as necessary and at least 30 minutes before bedtime. There is no maximum number of times during the day.
Encourage your child to drink plenty of fluids, cold or hot, depending on his or her preference.
Continue Tylenol or Tempra for throat pain and fever, but if the fever persists for 72 hours, or if his symptoms persist for 1 week without improvement, you will need to see a doctor.
