My 3 year old started to run a fever (40.5 to 41) 16 hours ago, but no other symptoms. What should I do?

Fever is calculated taken from the ears and 1.5 is added to the reading.

Hello Sir,
I highly recommend that you take your child's temperature rectally at least 1 time (if possible now) to really know the current value.
Then, at this age, we wait for 3 days of consecutive fever before consulting a doctor (especially in this period of cold and gastro). This is because his body may be fighting off a small virus and other symptoms may appear later (runny nose, cough).

Ok, right now he's sleeping, but I'm going to take it rectally when he gets up.
is it recommended to use Advil and tylenol alternately every 2 hours without exceeding the maximum daily dose for either?

I usually recommend splitting the doses by 3 hours to account for the fact that Advil™ is given every 6 hours. So I take advantage of the peak efficacy in the middle of the dosing interval.

Thank you very much. I'll get back to you with the rectal temperature

And you'll also tell me when the most recent Advil™ or Tylenol™ was taken when the temperature was taken! :)

This afternoon at 2:30 pm, rectal fever at 38.2 and Tylenol.
This evening, Tylenol taken at 6:30 pm (no fever taken (Bell Center)
fever taken at 10 pm at 41.5.

Thank you for those details.
In that case, you would need to see a doctor tomorrow Saturday if the fever is still there (it will be 3 days of fever).
In the meantime, continue the Advil and Tylenol, try to keep him resting well and not overdress him.
Happy day,
Alexander
