Child with swollen fingertips (just in front of the nail)?

Child with swollen fingertips (just in front of the nail). Not just one but all the fingers. The swelling has been stable for 4 days, maybe a little more important in the evening. A 2 year old boy who is currently undergoing pulmonary investigation. I wonder if it could be a sign related to his respiratory or circulatory functions.
