My son has a high fever, when should I seek medical attention?

My 2 year old boy has a big spike in fever since yesterday morning..if I don't give mitron and tylenol I'm not able to bring the fever down..no rash he tells me he doesn't hurt..besides roseola which I know has a big spike like that it could be what? Otitis but it would hurt? Strep but he would be in pain too? Thank you very much 🥺 I wonder if I'm exaggerating to want to consult ( note that I have 4 children and that normally despite even the COVID he didn't feel like this)

It is difficult to establish the exact cause of the fever. If it persists for more than 72 hours or if her general condition deteriorates, such as inability to drink or eat, you will need to seek medical attention.
For now, you can continue the Tylenol and encourage her to drink plenty of fluids and rest.
