My son has a high fever, when should I seek medical attention?

My 2 year old boy has a big spike in fever since yesterday morning..if I don't give mitron and tylenol I'm not able to bring the fever down..no rash he tells me he doesn't hurt..besides roseola which I know has a big spike like that it could be what? Otitis but it would hurt? Strep but he would be in pain too? Thank you very much 🥺 I wonder if I'm exaggerating to want to consult ( note that I have 4 children and that normally despite even the COVID he didn't feel like this) Okay thank you so much!
