If it is diaper rash (diaper irritation), I suggest applying a protective paste like Zincofax Extra Strength (40% zinc) at each diaper change, and checking the diaper often to change it as soon as it becomes the least bit dirty.

If it is really a localized problem in the vulva, I suggest to stop the cortisone cream and to consult a doctor as soon as possible, considering her age.
