Which cream should I use? Do I need a prescription or can a pharmacist give it to me?

I noticed a small patch this summer but it has since spread. I tried a low percentage cortisone cream found on the shelves which did not work. Since then I only use a moisturizer to calm the itching.

You did well with the use of moisturizer. I would be inclined to use an ointment such as Vaseline or Aquaphor as often as necessary for the skin of the feet, there is no maximum number of times during the day.
Avoid scented soaps, and after the bath or shower, avoid rubbing the skin vigorously: instead, lightly pat the skin to leave a thin layer of moisture, then apply the moisturizer or ointment within 5 minutes of getting out of the bath.
I invite you to try these measures for at least 2 weeks. If they are not successful, I suggest that you consult a physician as a precaution.
