How do I know if it's eczema and if so what to put on it? Should he see a doctor?

Hello ma'am, have you tried anything for this problem?
Is he complaining of itching or pain?

he only uses Cerave moisturizer. He complains of itching sometimes but not that much... could it be a reaction to the cold? He has really sensitive skin at the base but usually on his face.

Thanks for the feedback, please forgive the late reply!
Basically, the basis of treatment is moisturizing the skin and other non-pharmacological measures:
A fragrance-free moisturizer such as Aveeno, Cutibase, Base Glaxal, Cerave and others should be applied as often as needed. The cream should be applied whenever the need to scratch arises, among others.
If this only offers partial improvement, a more oily product such as Aquaphor can be tried.
Avoid scented soaps, and after the bath or shower, avoid vigorously rubbing the skin: instead, lightly pat the skin to leave a thin layer of moisture, then apply the moisturizer within 5 minutes of getting out of the bath.
In winter, make sure your child wears gloves and that his or her neck and face are well protected (scarf, neck warmer)
If non-pharmacological measures are insufficient, consult a physician for a thorough evaluation of your child's needs.
I hope this has been helpful and remain available if you have any further questions!
