Hello Ma'am,
It is common for small children to have ear pain. Indeed, it may be an ear infection. In general, the first 48 hours, we will only want to observe his condition since the majority of ear infections are viral in origin and go away by themselves . So we can give Tylenol or Advil in the meantime to soothe the child.
On the other hand, you should consult your doctor in the following situations:
Your child has a high fever and is in severe pain. (≥ 39°C)
His symptoms have worsened in the past 24 to 48 hours.
He is dizzy, has a severe headache, has a stiff neck, or his ear - or ear area - is swollen.
Fluid or blood is leaking from his ear.
His eardrum is perforated
