He is more tired, not really playing, no appetite in the last few days. I think it could be the teeth, because I've seen him put his in his mouth and chew on them, but how can I be sure it's not something else? The fever is high. Over 39.

First, sorry for the delay in responding.
In a situation like this, it is important to first relieve the child and keep the fever from rising too high. Have you used a product like Tempra™ or Advil™ for this?
Also, some type of virus infection typically gives a child bad breath. This can be a herpes virus or coxsackie (herpangina) infection. As a pharmacist, please know that I cannot diagnose this, but please know that these diseases do exist and can give children bad breath.
In all cases, one should check for fever and see if there are any pimples growing in the mouth, around the mouth, on the hands or feet.
