Hello,
In general, antibiotics start to work after 72 hours... You may not have a complete recovery after that time, but there should be an improvement that is noted. If your pain seems the same or worse than when you started, then it would be appropriate to reconsult your doctor; the penicillin may not be effective enough in this case.
I hope this answers your question... If there is anything else, please do not hesitate to contact me again.
Sincerely,
David Tan, pharmacist

If there is an "overall improvement," then you could wait until the end of your treatment and review the effectiveness of the antibiotic afterwards. In the meantime, you could still relieve the sore throat with the following measures:
Taking ibuprofen (Advil/Motrin) or acetaminophen (Tylenol) as needed...if there is swelling, ibuprofen will be better
Take sore throat lozenges (e.g. Cepacol extra strength) if needed
Gargle with water and salt for 1 minute 4 times a day
Drink a lot of water
Get plenty of rest
