Lithium is indeed eliminated by the kidneys, and its elimination is diminished in the presence of renal insufficiency. Usually the precaution for this is to start the medication at a lower dose, and if necessary to increase it more conservatively
In the vast majority of cases, the level of lithium in your blood will be measured.
