Unfortunately, medications in the Cipralex (Escitalopram) family do have a small tendency to cause sexual dysfunction. It is often libido, arousal and achievement of orgasm that can be affected while the feeling of satiety or satisfaction generally remains unchanged. Addressing lifestyle habits first, it is known that smoking and alcohol consumption can affect sex life while physical exercise would be beneficial. Doing 30 minutes of moderate intensity cardiovascular physical activity before sexual intercourse in addition to regular sessions 3 times a week can help with achieving orgasm. As for medications, escitalopram is most often implicated in sexual dysfunction and it would seem that this effect is also dose related. It is generally recommended that the medication be taken as far away from intercourse as possible, so if you are taking it in the morning, you should take it in the evening. Since you are already taking the minimum effective dose, switching your Cipralex to another drug in the same class would be the most effective option but I understand your reluctance. Generally, Wellbutrin, which you are taking, is a drug of choice for sexual dysfunction. These are changes that can be discussed with your pharmacist who may contact your primary care physician. If you are not comfortable with medication changes, there is some data on the use of Gingko Biloba, a natural health product, taken at 60-120mg once or twice a day. Side effects may include gastrointestinal discomfort and an increased risk of bleeding and bruising. Be sure to consult your pharmacist when choosing a product as doses are not standardized. The benefits are modest but may be worth a try. Best wishes, Jean-Félix Côté
