You did very well to give Benadryl and apply cortisone. You should continue to apply cortisone for a few days and give a few more doses of Benadryl as needed. In my opinion, you are already doing the right thing to treat this lesion. There doesn't seem to be any infection either. If it ever starts to stink, you would need to use an antibiotic ointment in addition, but we're not there yet.

She tells me that it hurts her bone
she has a really red back ear and it seems to be harder than the other ear.
should i consult just to make sure there is no stinger or the "bad guy" bug?
she tends to react a lot to the stings like swelling and stinging but i always get through it with benadryl, cortisone or polysporin...

I'm really annoyed. She says it hurts the bone. Probably she feels good pressure on the hard part behind the ears, but the ears are only made of cartilage. The bone would be the skull there. It's normal for it to be hard, if there's a lot of inflammation. For the infection, I am still not sure because the lesion is not hot
Before going to the doctor, you could perhaps try Bactroban ointment. It is more effective than Polysporin for skin infections. It's available over the counter at drug stores. If it spreads or the pain progresses despite applying the ointment 3 times a day, I suggest seeing a doctor in this case
