The risk is present, but very small. It is especially present if the fellatio was done while you had an "active" cold sore on your lip (not dried) and your spouse was not wearing a condom.

Background, cold sores are transmissible when...before the "button" appears, when there is stink in the "button" or when the lesion is open (example that one would have broken the "button")?

It is still transmissible. The risk is extremely low when there is no pimple on the skin and at its highest when the pimple is still wet.

No, you need an "active" lesion to be able to screen (and take advantage of it to find out if it is type 1 (cold sore) or type 2 (genital) herpes.
